Topic: What will the training be about?
Are meals provided (y/n):
Training for grade level(s): What grade level or levels will the training be appropriate for?
Facilitator's name: First and last name of the facilitator of the training
Time: When will this training take place
Mandatory training (y/n): State whether this training is mandatory or not
Paid training (y/n): State whether trainees will be paid or receive OPI renewal credits
Number of seats available: How many seats will be available at this training?
